The Withdrawl of the Rotashield Rotavirus Vaccination Due to an Association with Intussusception: Fact or Fiction?

چکیده

Rotavirus exists throughout the world and is estimated to have infected every individual by their fifth birthday. For this reason, there has been a significant drive for the development of a vaccination for the virus. After much failure, Rotashield, a tetravalent rhesus-based live attenuated vaccine was developed and approved in 1998. Following over 1.5 million administrations, it was suspended from the market in July 1999 and finally removed entirely in November of the same year due to fears of increased rates of intussusception following vaccination. Previous studies have shown a relationship between adenovirus infection and increased rates of intussusception. However similar relationships with wild-type rotavirus infection have not been demonstrated. Though studies have shown as much as a 25 times greater risk of developing intussusception within one week of vaccination, they fail to provide convincing conclusions. Past studies have either failed to include proper control groups which account for recency of wild-type rotavirus infection, consider cumulative rates of intussusception, or include sample sizes large enough to detect differences in intussusception rates.

More on RotaShield and intussusception: the role of age at the time of vaccination.

BACKGROUND RotaShield, a vaccine intended to prevent severe rotavirus diarrhea, was withdrawn in July 1999, 9 months after it became available in the United States, because of a temporal association with intussusception events that occurred in vaccinated infants. We explore here the effect of age on the risk of intussusception. METHODS We reanalyzed a case-control database of the Centers for ...

Relationship between Pentavalent Rotavirus Vaccine and Intussusception: A Retrospective Study at a Single Center in Korea

PURPOSE Despite withdrawal of RotaShield® and the development of second generation live attenuated rotavirus vaccines, concerns remain regarding the relationship between rotavirus vaccine and intussusception. Nevertheless, since there is no study in Korea, we reviewed data from cases at Severance Children's Hospital to determine the association between rotavirus vaccine and intussusception. M...

Rotavirus vaccines: an overview.

Rotavirus infection is the most common cause of severe diarrhea disease in infants and young children worldwide and continues to have a major global impact on childhood morbidity and mortality. Vaccination is the only control measure likely to have a significant impact on the incidence of severe dehydrating rotavirus disease. In 1999, a highly efficacious rotavirus vaccine licensed in the Unite...

Time to begin a new chapter and expand rotavirus immunization.

Rotavirus immunization has been limited to young infants owing to intussusception events noted with a prior rotavirus vaccine, RotaShield. Dramatic declines occurred in rotavirus-related disease beginning in 2008 after implementation of rotavirus vaccination in young infants. These declines occurred in vaccinated children and unvaccinated children and adults (through indirect protection). Despi...
